Scenario

As Designated Person Ashore (DPA) with Simply Shipping your role is to investigate incidents and ensure procedures are in place to prevent their re-occurance.

The MV Carry Cargo has just submitted a near miss report involving a passage planning and execution incident that could have led to the vessel going aground had the Master not intervened in a timely fashion.

DPA’S TASK

Your task, as Simply Shipping’s DPA, is to investigate the near miss and make recommendations to prevent recurrence of this type of incident.
