You’ve reached the rank of Master with Simply Shipping. The comapany’s Safety Management System (SMS) requires the passage plans of all vessels to be checked and approved (or amended as necessary) by the ship’s Master before commencing on the voyage.
As Master of the MV Carry Cargo your task is to check the passage plan from Southampton to Singapore and complete the SMS documentation with your findings.

As Designated Person Ashore (DPA) with Simply Shipping your role is to investigate incidents and ensure procedures are in place to prevent their re-occurance.
The MV Carry Cargo has just submitted a near miss report involving a passage planning and execution incident that could have led to the vessel going aground had the Master not intervened in a timely fashion.
Your task, as Simply Shipping’s DPA, is to investigate the near miss and make recommendations to prevent recurrence of this type of incident.
